I think we know what this means. Cloud secretly hated Aerith and drowned her, but when he saw how much his friends missed her, he warms up to her and regrets his decision, and that is why he so obsessed with Aerith's forgiveness in AC.

A good theory but I don't think so. I think of it in a way that Sephiroth's sword hit a vital artery [like the Aora for example]. I also read that the Masamune hits the soul not the body. I do find it strange that she died instantly while Cloud survived being stabbed though.

Aerith's death scene would've been better if there was an announcer behind it.
